2025 Annual Report Form

Neighborhood Associations are requested to complete this annual report form by December 31, 2025.  Responses will be compiled and shared with the City Council and will be used to help guide ongoing Council priority setting. Prior year reports are available here if needed for reference. 

Neighborhood Associations are also required to provide a signed Declaration of Spending Form to ensure they remain eligible for grant funding from the City.

Please complete the attached Annual Report Word form or webform below. Word is the preferred format if you have the software. Submit questions and/or completed Word forms to ddigby@westlinnoregon.gov . Thank you!
