Neighborhood Association Debit Card Application

The City of West Linn works in partnership with Pacific West Bank to provide each Neighborhood Association (NA) with a debit card account.  This is an important partnership, because it gives NAs the flexibility and freedom to use their city grant money for NA business.

There is a three step process that is required for adding any new debit card holders, or removing a debit card holder from the account prior to card expiration:

  1. Specifically receive authorization from your NA membership (in the form of an agenda item, a motion, and record in the minutes) for a debit card to be issued
  2. Complete the attached Pacific West Bank paperwork and return it to Pacific West Bank
  3. Complete the attached City of West Linn paperwork and return it to the City of West Linn
